You can use the form below to report any adverse events to us. Minor incidents (such as contact dermatitis or headache) are logged for internal records, while serious events are reported to the FDA in compliance with the Modernization of Cosmetic Regulations Act (MoCRA) of 2022.

Any personally identifiable information or medical detail provided in an adverse event report is protected under the Privacy Act of 1974 and will not be disclosed by the FDA.

Please include the following information: whether the Adverse Event is Minor or Major, your date of purchase, the name of the product, and a description of the Adverse Event's effects.
